The Idaho Person Centered Planning Project
- aims to empower people with disabilities to lead more self-directed lives by helping them create and harness the power of informal networks of support
- informal support networks include people such as community members, friends, and family who are dedicated to helping the person with the disability set and achieve goals
- Person Centered Planning Specialists assist individuals with disabilities create networks of informal supports and formulate personal goals, a plan to achieve those goals, and a process for implementing the plan
- is a collaborative effort between the Center on Disabilities and Human Development and the Idaho Council on Developmental Disabilities
- is a service delivery project funded by the Centers for Medicare & Medicaid Services
Training
- Person Centered Planning Specialists participated in an on-line training course from March 9-April 10, 2009
- an intensive face-to-face training took place in Boise from April 29-May 1, 2009
- follow-up training and support are being provided on a monthly and as needed basis
