The Idaho Person Centered Planning Project is a service delivery project funded by the Centers for Medicare & Medicaid Services. The Project aims to empower people with disabilities to lead more self-directed lives by helping them create and harness the power of informal networks of support. An informal support network includes people such as community members, friends, and family who are dedicated to helping the person with the disability set and achieve goals.
The Person Centered Planning Specialist assists the individual with the disability create his or her network of informal supports and formulate personal goals, a plan to achieve those goals, and a process for implementing the plan.
Affiliations
The Idaho Person Centered Planning Project is a collaborative effort between the Center on Disabilities and Human Development and the Idaho Council on Developmental Disabilities. Located within the University of Idaho, the Center on Disabilities and Human Development operates a variety of independent grant programs and carries out training, services, technical assistance, research, and dissemination activities across the state and nation. The Idaho Council on Developmental Disabilities’ purpose is to advocate for Idahoans with developmental disabilities to assure that they have access to needed community supports, individualized services and other forms of assistance that promote self-determination, independence, productivity, and inclusion in Idaho communities.
